Qualifications

• Bachelor’s degree in social work or related field.
• Active social work license issued by the Minnesota Board of Social Work or
• Bachelor's degree in special education, psychology, nursing, human services or other related fields.

Responsibilities

• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams.
• Connect individuals with community resources to promote independence.
• Maintain up-to-date documentation and compliance with guidelines.
• Complete daily case noting documentation of completed tasks.
• Travel to meet clients for annual/semi-annual meetings.
