Job summary

Thomas Allen in Minnesota is seeking a Case Manager – Waiver Services in Chisago County to develop comprehensive care plans, coordinate services, and connect individuals with community resources to promote independence. You will work with a diverse team, travel to meet clients for annual and semi-annual meetings, and provide education and guidance to individuals and families.

The role offers hybrid work after orientation.

Qualifications

• Bachelor’s degree in social work with Minnesota LSW license.
• One year of experience with developmental disabilities or a related course.
• Bachelor’s degree in special education, psychology, nursing, human services or related field.

Responsibilities

• Foster trusting relationships with individuals served.
• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams to ensure seamless delivery of care.
• Connect individuals with community resources to empower independence.
• Ensure compliance with lead agency guidelines and maintain meticulous documentation.
• Complete daily case noting of completed tasks.
• Collaborate with team to assess evolving needs and adjust plans.
• Provide support and guidance to individuals and families for informed care decisions.
• Travel to meet clients for Annual and Semi-Annual meetings and as needed.
