Responsibilities
- Foster trusting relationships with individuals served.
- Develop, review, and monitor individualized care plans.
- Coordinate services with interdisciplinary teams.
- Connect individuals to community resources.
- Ensure compliance and meticulous documentation.
- Complete daily case notes.
- Collaborate to adjust care plans as needs evolve.
- Educate and guide individuals and families.
- Travel to meet clients for annual/semi-annual meetings as needed.
