Job summary

Qualifications

• Bachelor’s degree in social work or related field and Minnesota license or eligible.
• One year of experience with developmental disabilities or a relevant course.
• Two years of full-time case management experience.

Responsibilities

• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams to ensure seamless delivery of care.
• Travel to meet clients in Southeast Minnesota for meetings and annual/semi-annual reviews.
• Maintain meticulous documentation and ensure compliance with lead agency guidelines.
• Provide support, education, and guidance to individuals and families to empower self-directed care.
