Job summary

Thomas Allen, Inc. is seeking a Waiver Case Manager in Chisago County to develop care plans, coordinate services, and connect clients with community resources. You will work to increase client independence and well-being, collaborating with a diverse team of health and social service professionals.

The role requires strong communication, empathy, and strong organizational skills. Hybrid work options are available after completing orientation and onboarding, with travel as needed to meet clients

Qualifications

• One year of experience in the education or treatment of people with developmental disabilities or related conditions, or completion of one relevant course.
• Bachelor’s degree in social work with an active Minnesota social work license.
• Bachelor's degree in special education, psychology, nursing, human services or related fields.

Responsibilities

• Foster trusting relationships with individuals served, emphasizing empathy, effective communication, and compassion.
• Develop, review, and monitor individualized care plans.
• Coordinate services and collaborate with interdisciplinary teams to ensure seamless delivery of care.
• Connect individuals served with suitable community resources, empowering their independence and well-being.
• Ensure compliance with lead agency guidelines and meticulous documentation.
• Complete daily case noting documentation of tasks.
• Collaborate with team to adjust care plans as needs evolve.
• Provide support and guidance to individuals and families for informed decisions.
• Regular travel to meet clients for Annual and Semi-Annual meetings.

Excellent written and verbal English

Strong organizational skills

Education

Bachelor’s degree in social work with active Minnesota social work license
