Thomas Allen, Inc. is seeking a Waiver Case Manager to support individuals in Southeast Minnesota from a home-based location. You will develop care plans, coordinate services, and connect clients with resources to enhance independence and well-being.

Travel is required to meet clients for regular annual and semi-annual meetings. The role emphasizes empathy, strong communication, and meticulous documentation, with licensing and relevant education as core prerequisites.

Qualifikationen

• Bachelor’s degree in social work with Minnesota license or a related field.
• 1 year of experience in education or treatment of developmental disabilities or a relevant course.
• Two years of full-time case management experience required.

Aufgaben

• Foster trusting relationships with individuals served with empathy and effective communication.
• Develop, review, and monitor individualized care plans.
• Coordinate services with interdisciplinary teams to ensure seamless care.
• Connect individuals with community resources to promote independence.
• Complete daily case noting documentation.
• Travel to meet clients for Annual and Semi-Annual meetings.
