Job summary

Shakir Consulting Services, LLC seeks a Waiver Case Manager in the Greater Minneapolis-St. Paul Area to coordinate services for waiver recipients, including older adults and people with disabilities.

The role is full-time, on-site/hybrid, and involves assessment, planning, and advocacy across social services, health care, and housing systems. You will document accurately, ensure compliance with county/state regulations, collaborate with families and providers, and uphold cultural responsiveness

Qualifications

• Experience in social services, case management, or related human services roles, preferably with waiver programs, older adults, or individuals with disabilities.
• Knowledge of community resources, housing supports, health and social service systems, and county/state regulations relevant to waiver and stabilization services.
• Excellent interpersonal, communication, and documentation skills for working with clients, families, community partners, and multidisciplinary teams.

Responsibilities

• Coordinate and manage services for individuals using waiver programs.
• Assess client needs and develop and implement service plans.
• Monitor service delivery and advocate for clients across social services, health care, and housing systems.
• Maintain accurate documentation and ensure compliance with county and state regulations.
• Collaborate with families and community providers and participate in team meetings and trainings.
