Job summary

Qualifications

• Experience in social services, case management, or related human services roles, preferably with waiver programs, older adults, or individuals with disabilities.
• Knowledge of community resources, housing supports, health and social service systems, and county/state regulations relevant to waiver and stabilization services.
• Excellent interpersonal, communication, and documentation skills for working with clients, families, community partners, and multidisciplinary teams.

Responsibilities

Role Description

The Waiver Case Manager is a full-time, on-site/hybrid role based in the Greater Minneapolis-St. Paul Area. In this position, the Waiver Case Manager coordinates and manages services for individuals using waiver programs, including older adults and persons with disabilities. Daily responsibilities include assessing client needs, developing and implementing service plans, monitoring service delivery, and advocating for clients across social services, health care, and housing systems. The role involves maintaining accurate documentation, ensuring compliance with county and state regulations, collaborating with families and community providers, and participating in team meetings and trainings. The Waiver Case Manager supports culturally responsive, person-centered care while upholding the agency's values and commitment to racial equity.

Qualifications

• Demonstrated experience in social services, case management, or related human services roles, preferably working with waiver programs, older adults, or individuals with disabilities.
• Previous waiver case management experience.
• Knowledge of community resources, housing supports, health and social service systems, and county/state regulations relevant to waiver and stabilization services.
• Strong skills in needs assessment, service planning, documentation, and compliance, with attention to detail and timely follow-through.
• Excellent interpersonal, communication, and conflict resolution skills for working with clients, families, community partners, and multidisciplinary teams.
• Commitment to cultural responsiveness, racial equity, and trauma-informed, person-centered practices in service delivery.
• Ability to work on-site, manage a caseload, prioritize tasks, and maintain professional boundaries while demonstrating empathy and respect.
• Proficiency in basic office software and electronic record-keeping systems; ability to learn agency and county documentation platforms.
• Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred; relevant experience may be considered in lieu of a degree where appropriate.
• Valid driver's license, reliable transportation, and ability to travel locally within the Greater Minneapolis-St. Paul and Ramsey County areas as needed.
