Job summary

Affinity Care MN is hiring a mission-driven Waiver Case Manager to deliver person-centered case management for individuals with serious mental illness, SUD, and co-occurring conditions across Hennepin County and surrounding metro. The role is based at the Corporate Office in Bloomington, MN, with travel within the metro area and non-exempt hourly pay of $25–$27 per hour.

Qualifications

• Must have one of the following: a bachelor’s degree in social work, psychology, sociology or related field; OR a bachelor’s degree in any field plus one year experience as a social worker/case manager.
• Must maintain an acceptable motor vehicle record and have a valid driver's license with a personal insured vehicle for travel.
• Proficiency in office productivity software and strong written and verbal communication.

Responsibilities

• Carry and maintain a caseload of individuals with serious mental illness, SUD, and co-occurring conditions.
• Meet with clients regularly per waiver-specific rules and timelines.
• Manage intake and discharge within established parameters.
• Collaborate with physicians, mental health providers, and care teams to update each client’s plan.
• Ensure financial authorizations and provider contracts are in place.
• Document services accurately to track outcomes and maintain records.
• Connect clients with community and housing resources matching needs and preferences.
• Provide crisis assessment and intervention to keep clients safe.
