Job summary

Paradigm seeks a Vocational Case Manager to cover the Chicago area, coordinating return-to-work plans for injured workers and conducting comprehensive vocational assessments to identify suitable employment opportunities.

The role requires a Bachelor's in Rehabilitation Counseling or related field and credentials like CRC/CCM/CDMS; travel is involved. You will collaborate with clinicians, insurers, employers, and attorneys in a fast-paced, supportive environment.

Qualifications

• Bachelor's degree in Rehabilitation Counseling or related field with course work in rehabilitation philosophy, history, ethics, medical aspects of disability and job placement.
• CRC, CCM, or CDMS certification required.
• Experience in providing Vocational Rehabilitation Services.
• Valid driver's license and good driving history.
• Knowledge of Managed Care or Workers' Compensation preferred.
• Knowledgeable of state and local regulations.
• Excellent communication skills, both verbal and written.
• Ability to interface professionally with claims staff, clients, employers, attorneys, physicians and their representatives.

Responsibilities

• Schedule appointments and meet with clients, families, insurance adjusters, medical providers, employers and others as needed.
• Assist and coordinate return-to-work planning including job analyses and modifications.
• Coordinate with treatment providers to determine physical capabilities and return-to-work readiness.
• Complete Vocational Assessments gathering medical, social, economic, education, and work history data.
• Develop Rehabilitation/Case Management Plans tailored to client needs.
• Provide job development and placement services and resume/interview coaching.
• Document all case activity and maintain organized reports within guidelines.
