Job summary

Qualifications

• Bachelor of Science in Nursing or Health Care Administration required.
• Master’s degree preferred.
• Minimum 5 years in health care management with at least 3 years of leadership.
• Minimum 1 year caring for frail or elderly populations.
• Valid NC driver’s license and reliable vehicle.
• Ideally PACE program experience.

Responsibilities

• Lead Center Managers, On Site Services Liaison, Community Support Liaison and staff as designated.
• Ensure decisions regarding medical, social, physical, and emotional support follow IDT processes.
• Provide oversight of clinical budget expenses and resource utilization.
• Facilitate IDT care planning and lead family/team meetings as needed.
• Ensure 24-hour care coordination and back-up coverage as required.
• Ensure compliance with state, local, and CMS regulations and standards.
