Job summary

Included Health seeks board certified Nurse Practitioners licensed in Kentucky to deliver virtual comprehensive care to patients of all ages. This role requires 3+ years in high‑volume Urgent Care or Primary Care and active RN licensure with multi‑state credentials.

We offer two shift options: Modified Full‑Time (32 hours/week) or Part‑Time (20 hours/week), with evenings and weekends plus two holiday shifts.

Qualifications

• Graduate of an accredited Master’s or Doctoral‑level Family Nurse Practitioner (FNP) program.
• Active Nurse Practitioner (NP) license with RN licensure in accordance with state requirements.
• Current national certification as a Family Nurse Practitioner through AANP or ANCC.
• Active RN compact license.
• Active multi‑state licensure (5+ licenses preferred).

Responsibilities

• Conduct age‑appropriate history and virtual exams.
• Evaluate, diagnose, and treat a broad range of conditions across the lifespan.
• Order and interpret diagnostic tests as needed.
• Develop and document treatment plans in a timely, accurate manner.
• Manage acute, chronic, and mental health conditions for pediatric and adult patients.
• Request consultation or referral with other health care providers when appropriate.
• Counsel and educate patients and families on treatment plans.
• Maintain current knowledge of clinical best practices and evidence‑based care.
• Provide cross‑coverage for tasks such as reviewing lab results.
• Offer scheduling flexibility, including evenings and weekends.
