Job summary

Included Health, Inc. is seeking board-certified Nurse Practitioners licensed in New Hampshire and other states to deliver virtual comprehensive care to patients of all ages, in urgent and primary care settings. The role requires at least 3 years of high-volume experience and active multi-state NP/RN licensure.

Ideal candidates will have telemedicine experience and Athena EMR proficiency. Part-time (20 hrs/wk) with evenings and weekends, plus productivity incentives, are offered.

Qualifications

• Graduate of an accredited Master’s or Doctoral-level FNP program.
• Active NP license with RN licensure in accordance with state requirements.
• Current national FNP certification (AANP or ANCC).
• Active RN compact license; multi-state licensure preferred.
• 3+ years post-training in high-volume Urgent Care or Primary Care.
• Experience caring for newborns through adult populations.
• Ability to function in an integrated medical practice.
• Strong clinical acumen and broad condition management.
• Excellent communication and patient engagement skills.
• Maintenance of Medicare status and not excluded from federal programs.

Responsibilities

• Conduct age-appropriate history and virtual exams.
• Evaluate, diagnose, and treat a broad range of conditions across the lifespan.
• Order and interpret diagnostic tests as needed.
• Develop and document treatment plans in a timely, accurate manner.
• Manage acute, chronic, and mental health conditions for pediatric and adult patients.
• Request consultation or referral with other providers when appropriate.
• Counsel and educate patients and families on treatment plans.
• Maintain knowledge of clinical best practices and evidence-based care.
• Provide cross-coverage for tasks such as reviewing lab results.
• Offer scheduling flexibility, including evenings and weekends.
