Qualifications

• Board Certified as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in Michigan.
• Professionalism, integrity, confidentiality and compassionate patient care.

Responsibilities

• Conduct virtual wellness visits for adults and/or pediatrics.
• Perform health risk assessments and medication reviews.
• Coordinate with in-home clinical team to visit patients and arrange tests.
• Provide patient education and comprehensive care coordination.
