Qualifications

• Board-certified Family Nurse Practitioner with active license.
• Ability to obtain or hold licenses in applicable states.

Responsibilities

• Review case load and prepare tools for patient visits.
• Perform virtual wellness visits and health risk assessments.
• Coordinate with Sprinters for in-home visits when needed.
• Conduct medication reviews and cognitive screenings.
• Provide patient education and health coaching.
• Document findings and coordinate referrals to specialists.
• Ensure HIPAA-compliant care coordination.
