Job summary

CareTalk Health is a fully remote, nationwide telehealth practice partnering with Medicare Advantage plans to deliver Annual Wellness Visits and close care gaps. As an RN or LPN/LVN, you will prepare comprehensive AWV charts, review patient histories, medications, and screenings; conduct virtual visits; and communicate concise summaries to providers.

This role requires an active nursing license and 3+ years of clinical experience, strong EHR and telehealth proficiency, and a dedicated private

• Ability to maintain a private home workspace with reliable internet.
