Job summary

Capital Health is seeking a Violence Intervention Program Coordinator to lead hospital-based violence prevention efforts and manage a complex grant program across the health system. Qualifications

  • High School Diploma or equivalent.
  • Equivalent college coursework in social services field preferred.
  • Three years case management experience required.
  • Project management and data analytics experience preferred.
  • AHA BLS. Non-Licensed must obtain before end of orientation period

Responsibilities

  • Uses information technology in accessing, collecting, analyzing, maintaining and disseminating data and information.
  • Evaluates the integrity, validity and reliability of data with the goal of analysis, trend identification, and gap resolution.
  • Examines the feasibility (ex. fiscal, social, political, legal) and implications of policies, programs and services.
  • Recommends, implements and evaluates policies, programs and services in collaboration with Institute of Urban Care Manager.
  • Provides intensive case management, mentoring and advocacy to target population involved in violence.
