Colonial Terrace Animal Hospital, P.c. is seeking a Patient Care Coordinator to be the central liaison for clients from check-in to check-out. You will document medical notes, assist in forming treatment plans, and coordinate all aspects of patient care with our veterinary team.

You will educate clients, manage discharge instructions, and ensure prescriptions are fulfilled. A friendly, empathetic demeanor and strong communication are essential for delivering exceptional client service in our

Qualifications

• High school diploma or equivalent.
• Love for animals and exceptional client service.
• Friendly, personable, and empathetic demeanor.
• Proficient in active listening, with strong computer literacy and typing skills for accurate medical transcription.
• Excellent written and verbal communication skills.
• Weekend urgent care shift required per month.

Responsibilities

• Guide clients through visits from check-in to check-out.
• Capture medical notes and coordinate care with the veterinary team.
• Create, present, and explain treatment plans to clients.
• Coordinate care and ensure timely completion of plans with the team.
• Handle in-room check-outs, payments, and scheduling follow-ups.
• Educate clients and explain discharge instructions; ensure prescription fulfillment.
• Conduct post-visit follow-ups to check pet well-being.
