Job summary

Socket.dev in Hampton, VA is seeking a Community Outreach Case Manager to build partnerships and identify individuals needing services for veterans and families. Qualifications

• Bachelor’s degree in Human Services or related field.
• 3 to 5 years of outreach experience with Veterans or mental health/substance use concerns.
• Ability to explain agency programs and operations.
• Knowledge of local community resources.
• Self-starter with mission-driven approach to connecting people with services.
• Excellent computer skills in Microsoft Office.
• Excellent written and verbal communication.
• Conflict resolution skills; bilingual is a plus.

Responsibilities

• Coordinate outreach efforts for the Veterans Program.
• Develop outreach plans to target eligible participants and schedule visits.
• Screen potential program participants for eligibility.
• Provide or arrange SSG Fox SPGP services as outlined in guidelines.
• Outreach to identify those at risk of suicide.
• Baseline mental health screening for participants 18+.
• Educate on suicide risk and prevention for families and communities.
• Provide case management and peer support services.
• Assist with VA benefits and coordinating other benefits.
• Travel throughout the community; engage with homeless neighbors until housing is found.
• Maintain client records and activity/mileage logs.
• Attend meetings and trainings; participate in supervision.
• May drive a van and conduct vehicle safety inspections.
