Qualifications

• Bachelor's degree preferred in social services fields
• 1+ years case management experience; 3+ years preferred; 2+ years in a customer service environment
• Knowledge of local social services, public health, and VSO resources is a plus
• Experience with military or veteran populations preferred
• Familiarity with HMIS is preferred
• Excellent written and oral communication skills; highly organized and reliable

Responsibilities

• Provide comprehensive case management for veterans seeking housing and related services
• Interview veterans to determine housing and social service needs
• Provide appropriate referrals as necessary
• Prepare and maintain confidential case records
• Collaborate with service providers and community partners on resource issues
• Ensure confidentiality and access control of information
• Coordinate with clinical team for risk management and crisis intervention as needed
• Develop a network of referrals for care coordination
• Participate in multidisciplinary team meetings
• Follow up to ensure services are received and follow-up needs are met
• Assist veterans in navigating multiple care systems
• Maintain case information in VVSD EHR
• Document client information in HMIS
