Job summary

Nation’s Finest is a Veterans’ services nonprofit delivering housing, health, and employment support across multiple sites. The Case Manager helps participants achieve stability, wellness, and long-term self-sufficiency through client-centered case management, outreach, housing navigation, and employment support.

The role includes intake screenings, service plan development, advocacy, and coordination of financial assistance, health benefits, and referrals, in both office and community settings.

Qualifications

• Minimum 1 year of experience in case management, housing navigation, or related social services.
• Knowledge of social services, housing programs, employment resources, and crisis intervention strategies.
• Ability to manage administrative tasks across multiple funding streams with compliance.
• Proficiency with EHR systems and documentation standards.
• Commitment to trauma-informed, client-centered, and culturally responsive practices.

Responsibilities

• Conduct client screenings, intakes and assessments to determine eligibility and goals.
• Develop, implement, and monitor service plans in collaboration with clients.
• Provide advocacy, goal setting, crisis intervention, and progress monitoring.
• Coordinate supportive services including financial assistance and health benefits.
• Educate clients on program expectations and adherence to rules and policies.
• Engage in community outreach to connect clients with resources and referrals.
• Maintain complete, confidential case files and report outcomes for compliance.
• Collaborate with staff and partners to ensure holistic, integrated care.
