The Case Manager will support participants in achieving stability and self-sufficiency through client-centered services, housing navigation, and employment support in a full-time role.

The role emphasizes compassionate engagement, collaboration with partners, and maintaining compliant documentation across funding streams.

Qualifications

• Minimum 1 year of experience in case management or housing/navigation
• Bachelor’s degree in social work, psychology, or related field strongly preferred
• Knowledge of social services, housing programs, employment resources, and crisis intervention strategies
• Strong interpersonal, communication, and organizational skills
• Experience with multiple funding streams and program reporting compliance
• Proficiency with EHR systems and client-tracking databases
• Understanding of program budgeting, cost allocation, and performance outcome tracking
• Commitment to trauma-informed, client-centered practices
• Veteran status is desirable but not required

Responsibilities

• Conduct client screenings, intakes and assessments to determine eligibility, needs, and goals
• Develop, implement, and monitor service plans in collaboration with clients
• Provide strengths-based case management including advocacy, goal setting, crisis intervention, and progress monitoring
• Coordinate services with staff and external partners to ensure holistic care
• Educate clients on program expectations and support adherence to rules and policies
• Engage clients through outreach in communities and service sites
• Build relationships with landlords, housing providers, and community agencies
