Job summary

Veterans Transition Center in the United States is seeking a HCHV Case Manager to coordinate assessment, treatment planning, and follow-up for Veterans in an emergency shelter program. You will advocate for Veterans’ rights, work with a multidisciplinary team, and support up to 20 clients in a fast-paced environment.

The role emphasizes documentation accuracy, knowledge of VA systems, and coordination with housing and program staff to ensure successful outcomes.

Qualifications

• Degree in human services field or two to five years related experience.
• Valid state driver's license and access to a vehicle.
• Experience with VA/HUD-VASH or related programs preferred.

Responsibilities

• Meet with assigned residents at least once a week.
• Coordinate and schedule the interdisciplinary treatment planning meeting and write plans.
• Document weekly progress and maintain client records.
• Provide crisis intervention and crisis phone coverage.
• Coordinate benefits applications (VA, SSI, Medicaid, etc.).
• Assist Veterans in obtaining employment or education/training.
