Valor Healthcare is seeking a Primary Care/Mental Health Integration LCSW for the Community Based Outpatient Clinic in Bowling Green, KY. The role offers a competitive salary, excellent benefits, generous time off, and a weekday schedule.

PCMHI clinicians provide brief, solution-focused interventions for veterans and collaborate with the VA in a multidisciplinary team. Final compensation will reflect experience, VA requirements, and clinic needs.

Qualifikationen

• Master’s degree in Social Work or Doctoral degree in Psychology.
• Must be credentialed and in good standing with the VA.
• Minimum of 2 years post-master clinical counseling experience.
• Licensed LCSW/MSW (or equivalent) in the state of employment.
• Proficiency in written and spoken English.
• Strong computer skills including EMR experience.

Aufgaben

• Performs initial and follow-up evaluations, including assessments.
• Engages veterans, core and expanded care team in treatment planning.
• Provides a range of treatment services, including individual therapy.
• Offers evidence-based psychotherapies as needed.
• Provides crisis intervention and coordinates care with family and other services.
• Inputs patient information into VISTA/CPRS and maintains confidentiality.
• Collaborates with interdisciplinary clinical teams to coordinate care.
• Participates in program planning and clinic improvement initiatives.
