Job summary

Valor Healthcare is seeking a licensed Master Social Worker (LMSW) to join our team at the VA Community Based Outpatient Clinic (CBOC) in Bowling Green, KY on a part-time basis. You will work with an all adult veteran population within our primary care/case management team, delivering compassionate psychosocial support and resources.

Qualifications

MSW degree from a program registered by the Department of Education.
Active MSW license in the state of employment or VA contract requirements.
1–3 years post-master direct counseling experience with adult patients.

Responsibilities

Complete psychosocial assessments to identify underlying causes.
Assist patients and families in understanding factors and pros/cons of solutions.
Coordinate with clinic staff and community agencies for problem solving.
Address Advance Directives and Organ Donation per VA standards.
Document social work interventions in CPRS and ensure proper hand-off.
Support enrollment, outreach, and retention efforts for appointments.
