Valor Healthcare, Inc. seeks a passionate Primary Care/Mental Health Integration LCSW to join the Community Based Outpatient Clinic in Harriman, TN. This part-time position focuses on veterans and provides brief, solution-focused interventions within a primary care setting.

The PCMHI clinician will participate in a collaborative care model, perform evaluations, provide individual therapy, and coordinate with the VA and clinic team to support mental and physical health outcomes for veterans.

Qualifications

• Must have a valid LCSW/MSW license in the state of employment.
• Experience delivering mental health services in adult populations.
• English language proficiency required for clinical documentation and communication.

Responsibilities

• Performs initial and follow-up evaluations.
• Engages veterans and treatment teams in planning.
• Provides a range of psychosocial interventions, including individual therapy.
• Coordinates care with VA and clinic staff to support mental and physical health outcomes for veterans.
• Maintains confidentiality and completes clinical reminders and documentation in the EMR.
• Participates in outcomes improvement and compliance with VA/Valor policies.
