Job summary

Valor Healthcare seeks a compassionate Primary Care/Mental Health Integration LCSW to join our Community Based Outpatient Clinic in Cookeville, TN. The PCMHI model expands mental health access within primary care settings and requires VA training/certification.

The clinician will provide brief, focused therapy for veterans and coordinate care with the PACT team. The successful candidate will conduct assessments, engage the care team, and deliver short-term therapies while upholding VA

Qualifications

• Must hold a current LCSW license (TN).
• Experience with adult patients in outpatient settings preferred.
• Familiarity with PCMHI or integrated care is a plus.

Responsibilities

• Perform initial and follow-up assessments.
• Engage veterans and care team in treatment planning.
• Provide individual therapy and brief, solution-focused interventions.
• Deliver evidence-based psychotherapies as appropriate.
• Coordinate crisis intervention and emergency referrals.
• Maintain confidentiality and accurate EMR documentation.
• Collaborate with interdisciplinary team to optimize care.
