Job summary
Great Plains Veterans Services Center in Fort Belknap, Montana is seeking a Veterans Case Manager to provide outreach, intake, assessment, and comprehensive case management for area Veterans. You will assist Veterans in obtaining military records and earned benefits, develop individual service plans, and coordinate with GPVSC and community partners.
The role emphasizes suicide-prevention and wellness activities, adherence to federal, tribal, state, and grant requirements, and maintaining
Qualifications
- Three years of experience in social services outreach or related work.
- Preferred formal education in social work, sociology or psychology.
- Ability to document decisions and maintain confidential records.
Responsibilities
- Conduct outreach, intake, assessments to identify needs and priorities.
- Develop and monitor individualized service plans and follow-up.
- Coordinate with VA, tribal, state and local resources.
- Advocate and liaison between Veterans, families, GPVSC staff and partners.
- Maintain accurate and confidential case records.
