Job summary

Axiom Care, a Phoenix-based behavioral health organization, seeks a Veterans Case Manager to support transition toward recovery and community reintegration. You will run therapeutic groups, coordinate care with clinical and housing staff, and manage referrals to ensure seamless service delivery.

Responsibilities include documenting sessions, developing client-driven treatment plans, and maintaining EMR records.

Qualifications

• Minimum one year of case management experience with AHCCCS population or similar.
• Experience coordinating internal and external care and referrals.
• Proficient with EMR systems and Microsoft Office.

Responsibilities

• Facilitate and document group sessions focused on recovery and life skills.
• Coordinate internal and external client care to align with treatment goals.
• Maintain accurate electronic medical records and client communications.
