Axiom Care is a Phoenix-based behavioral health organization with locations in the Phoenix metro area and Apache Junction. The Case Manager – AJ Detox role focuses on housing-first, high-need case management for justice-involved individuals and those with serious mental illness, within a sober living setting.

You will maintain HIPAA compliance, develop client-centered treatment plans, manage a caseload of about 20–30 clients, document within 24 hours, and collaborate with an interdisciplinary

Qualifications

• Minimum one year of case management knowledge with AHCCCS population, justice-involved individuals, chronically underhoused and substance use treatment.
• Experience with crisis/SMI population preferred.

Responsibilities

• Maintain strict adherence to state and local ethical standards and legal requirements at all times.
• Complete case management functions focused on housing-first and high-needs case management model.
• Assist clients with AHCCCS applications and other paperwork to aid financial stability.
• Develop client-driven individualized treatment plans and monitor progress.
• Carry a caseload of about 20-30 clients through their residential stay.
• Document all services and follow-up within 24 hours.
• Collaborate with external resources to coordinate care.
• Ensure HIPAA compliance and data security.
• Attend clinical and interdisciplinary team meetings as requested.
• Maintain open communication with tech staff and leadership.
• Identify maintenance and safety issues at the facility.
• Intervene with de-escalation and emergency response when needed.
