Qualifications

• Requires completion of nursing assistant training or certification.
• Ability to work effectively within a care team.
• Willingness to work rotating shifts and holidays as required.

Responsibilities

• Assist licensed nursing staff in caring for patients/residents across preventive, acute, sub-acute, chronic and hospice settings.
• Enter resident information into charts and participate in care planning and infection control.
• Serve as mentor to other CNAs and participate in treatment planning meetings.
• Perform basic nursing procedures within scope of practice and according to facility policies.
• Monitor intake/output, document observations, and recognize subtle changes in patient condition.
• Provide direct patient care including glucose monitoring and routine assessments as needed.
