Job summary

The VA Western New York Health Care System seeks a Social Worker Admissions Coordinator for the Community Living Center (CLC) in Buffalo. You will provide direct social work services to veterans and families and serve as the admission coordinator for CLC admissions.

• Coordinate and evaluate appropriateness of CLC admissions with the Medical Director and Nurse Leader.
• Lead the discharge plan and recommend home- or community-based care.
• Serve as liaison with the interdisciplinary treatment team for veteran respite opportunities.
• Assist with intake, screening, and timely disposition of veterans.
• Provide case management and education to veterans and families regarding VA benefits.
