Job summary

Qualifications

• Must have a master's in social work from CSWE-accredited program.
• Must be licensed or certified by a state to independently practice social work.
• Active pursuit of licensure/certification if not yet licensed.

Responsibilities

• Identify homeless Veterans and enroll them for VA Health Care services.
• Conduct psychosocial assessments to determine needs of Veterans and families.
• Participate in discharge planning with other disciplines to ensure timely, appropriate plans.
• Provide individual, family and group counseling as appropriate.
• Consult with Veterans and families on resources, directives, and VA benefits.
• Serve as liaison to community resources and programs to support homeless Veterans.
