Job summary

U.S. Department of Veterans Affairs is seeking a Social Worker at the GS-12 level to provide comprehensive psychosocial care to Veterans. Duties include discharge planning, crisis counseling, and coordination with an interdisciplinary team to meet medical and social needs.

Position offers weekend coverage and non-telework status in a VA medical center environment. The role requires an MSW from a CSWE-accredited program, state licensure, and documented experience in advanced practice.

Qualifications

• Must have a master's degree in Social Work from a CSWE-accredited program.
• Must be licensed to independently practice social work at the master's degree level.
• Two years of post-advanced licensure experience in a specialized social work area; at least one year at GS-11 level.
• English proficiency in speaking and writing as per policy.
• Advanced practice social workers must be licensed/certified and able to provide supervision for licensure.

Responsibilities

• Provide coverage for Inpatient Units, ICU, Emergency Department and Outpatient Services on weekends.
• Ensure continued discharge planning for Veterans with coordinated discharge.
• Conduct psychosocial assessments and plan pre-discharge and admission strategies.
• Develop treatment plans and provide counseling services to Veterans and families.
• Coordinate care with a multidisciplinary team and community resources.
• Perform suicide risk screenings and clinical reminders; ensure safety plans are in place.
