Qualifications

• Master's degree in social work from CSWE-accredited program.
• Licensed or certified to independently practice social work at MSW level.
• Experience providing psychosocial assessments and treatment to veterans.

Responsibilities

• Provide case management and care coordination for Veterans in assigned programs.
• Conduct psychosocial assessments and develop treatment plans.
• Coordinate with interdisciplinary teams to deliver psycho-social care.
• Refer Veterans to community resources and VA benefits.
