Qualifications

• Must have completed an approved nursing program and be eligible for RN licensure.
• Proficiency in English (written and spoken).
• Active RN license in state where employed or eligibility to obtain license.
• BSN preferred; licensure via bridge program or equivalent acceptable.
• Ability to coordinate care and educate Veterans throughout the care continuum.

Responsibilities

• Execute a streamlined approach to receiving, triaging, and directing timely, coordinated care.
• Coordinate care and educate Veterans to support self-management and satisfaction.
• Liaise with internal and community providers to manage Veteran's care journey.
• Assess, plan, implement, and evaluate care to Veterans in the community.
• Support delivery of high-quality, timely care and patient education.
