Job summary

Diversity Group seeks a dedicated registered nurse to oversee nursing processes and supervise direct patient care within community-based Veteran services. You will assess, plan, implement, and evaluate care, coordinating education, self-management, and patient satisfaction across the continuum.

Responsibilities include triage of consults, expediting referrals, and liaising with providers to ensure timely, high-quality care. Must hold an approved nursing degree and active state license.

Qualifications

• Proficient in English written and spoken.
• Graduate of a nursing program approved by ACEN/CCNE or equivalent.
• State license required upon appointment or during bridge program.
• Foreign-educated nurses require degree equivalency validation.

Responsibilities

• Assist in triaging consults/referrals and expedite clinical needs.
• Review community care documents and coordinate timely in-network referrals.
• Provide community providers with referrals and authorizations compliant with standards.
• Liaise between referring providers and specialty care services.
