Job summary

Diversity Group is seeking a Home Based Primary Care Outpatient Staff Registered Nurse to deliver comprehensive, knowledge-based care to veterans in their homes. The role includes care coordination, patient education, and collaboration with an interdisciplinary team to optimize outcomes.

The nurse will conduct assessments, plan and implement care, and ensure adherence to policies while addressing geriatric and complex needs in a home-based setting.

Qualifications

• English proficiency is required
• State nursing licensure in US jurisdictions preferred
• Nursing degree from ACEN/CCNE accredited program preferred
• Experience in home-based primary care or geriatrics is desirable

Responsibilities

• Provide direct Veteran care and care coordination within the HBPC program
• Perform case management, assessment, triage, education, resource coordination, and care planning
• Deliver technically competent nursing care in the home setting and collaborate with interdisciplinary team
