Job summary

Renown Health in Reno, NV seeks a Nurse Navigator to coordinate care and provide education to patients and their families. You will work with the multidisciplinary team to implement care plans, facilitate referrals, and support discharge planning and telehealth.

Must have RN license in Nevada, Bachelor’s preferred, with BLS/ACLS and Microsoft Office proficiency. Three years acute care nursing experience preferred, strong communication and program development skills.

Qualifications

• Bachelor’s degree in Nursing preferred.
• Three years acute care nursing experience preferred.
• Current Nevada RN license required at time of hire.
• Current BLS by AHA standards required at time of hire.
• ACLS by AHA standards required at time of hire; advanced cardiac certification preferred within two years of hire.
• Proficient with Microsoft Office Suite (Outlook, PowerPoint, Excel, Word).

Responsibilities

• Coordinate care and educational support to patients and families.
• Lead or participate in programs that support clinical excellence.
• Coordinate multidisciplinary care plans and referrals.
• Support discharge planning and telehealth appointments as needed.
• Develop and evaluate evidence-based protocols and participate in committees.
