Join the Sioux Falls VA Health Care System as a Referral Clinical Coordinator Registered Nurse to help Veterans access timely, high-quality care through community providers. You will coordinate referrals, manage complex care needs, collaborate with interdisciplinary teams, and advocate for Veterans throughout their care journey.

You will improve access to services, support informed decisions, and ensure seamless transitions back to VA care, while contributing to the Veteran-centric mission of VA.

Competenze

• RN license (current, active, and unrestricted) per VA requirements.
• BSN preferred or bridge programs considered for licensure pathways.
• Experience in care coordination or case management is valued.
• Ability to advocate for Veterans and work with interdisciplinary teams.

• Coordinate referrals to community providers to ensure timely access to care.
• Manage complex care needs and support transitions between VA and community services.
