Responsibilities
- Accountable for all elements of the nursing process (assessment, diagnosis, planning, implementation, evaluation)
- Coordinate comprehensive care for Veterans focusing on education and self-management
- Triages referrals and directs timely, coordinated care per VA standards
- Act as liaison between internal and external providers to ensure access to VA and community care
- Contribute to quality improvement and evidence-based practice initiatives
