• 1 year of relevant experience in a clinical setting

Responsibilities

• Provide basic patient care including comfort, vitals, personal care and mobility assistance
• Act as liaison between patient and nurse to report changes or concerns
• Provide accurate documentation of patient care in the health records
• Participate in hand-offs and reporting of patient needs
• Mobilize patients using safe transfer techniques and encourage independence
• Personalize care based on patient interests and needs
