Job summary

U.S. Renal Care is seeking a regional clinical operations leader to oversee nursing and care management across multiple dialysis facilities. You will coach Nurse Case Managers, Navigators, and Coordinators, implement clinical initiatives, and ensure high-quality patient outcomes in value-based care.

Travel 20-50% is required; you will partner with physicians, facility leaders, and HR for staffing, credentialing, and workflow optimization, driving performance metrics and continuous improvement.

Qualifications

  • Clinical operations background required.
  • Advanced Practitioner degree or Bachelor’s in Nursing preferred.
  • Minimum of 5 years nursing leadership experience across multi-location healthcare.
  • Proficient with Microsoft Office products (Outlook, Word, Excel).
  • Strong analytical skills and experience in program development.
  • Knowledge of Health Information Exchange (HIE) and EMRs preferred.

Overview

Lead a regional team of clinical team members to drive quality patient outcomes in value-based care. Oversee operations, staffing, compliance, and workflows while partnering with physicians and dialysis facilities. Monitor performance metrics, implement process improvements, and talent management. Collaborate closely with cross-functional colleagues to develop and execute

Responsibilities

Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.

  • Manages and coaches the clinical team
  • Directly manages a team of Nurse Case Managers, Navigators, and Coordinators, providing direction, mentoring, and professional development.
  • Leads implementation of new clinical initiatives through change management, stakeholder engagement, education, and ongoing performance monitoring
  • Coaches and mentors the team through regular team meetings, on-site shadowing, and other means.
  • Conducts regular 1:1 meetings, team huddles, and comprehensive team meetings to ensure alignment and address concerns
  • Routinely reviews team metrics / scorecards with individual team members and the full team
  • Ensures optimal deployment and daily productivity of the clinical team, including by balancing caseloads and assignments
  • Supports nurses in optimizing their schedules and patients (e.g., prioritizing on-site work for nurses who have in-person responsibilities)
  • Oversees payroll, PTO, expense management, and other Human Resources processes, ensuring accuracy and compliance
  • Addresses staffing needs by liaising with Human Resource Business Partners, conducting interviews, overseeing onboarding, and precepting new hires
  • Monitors compliance with professional licenses and manages credentialing processes
  • Is routinely in the field with the team to provide in-person coaching and mentoring; regular travel required when not geographically close to team and partners
  • Drives high-quality clinical care and partnership with other clinician and non-clinician partners
  • Designs, implements, and continuously improves standardized clinical workflows to improve efficiency, consistency, and patient outcomes across multiple partners. This includes regular reviews of Optimal Starts patients, regular reviews of CVC patients, engagement with dialysis facilities, IDT meetings for key physician practices, and others as needed
  • Partners with Associate Chief Medical Officer, Dialysis Clinic Leaders, and Nurse Case Managers to build strong care plans, removes barriers to execution, and ensures timely delivery.
  • Partners with Market Operators to design and deploy workflows with each relevant physician practice and dialysis team
  • Ensures routine engagement of Nurse Case Managers, Navigators, and Coordinators with Risk Adjustment Nurse Practitioners to deliver high-quality care
  • Develops close working relationships with dialysis facility administrators, and ensures that appropriate communication processes and other workflows are in place to maximize impact of interventions for dialysis patients
  • Develops close relationships with physician practice leaders and administrators, and is seen as a key clinical resource to drive impact with the practice
  • Provides oversight of clinical practice standards
  • Analyzes clinical performance data to identify opportunities, establish action plans, monitor progress, and drive continuous improvement across key metrics
  • Leads reporting in Monthly Operating Reviews on clinical team performance and issues
  • Ensures evidence-based interventions are consistently utilized
  • Monitors quality indicators and develops corrective action plans
  • Leads design and implementation of new care management approaches
  • Evaluates emerging practices and recommends clinical program enhancements
  • Leads population health strategies across assigned markets utilizing predictive analytics, risk stratification, and targeted interventions.
Key Performance Indicators

KPIs include:

  • Optimal Dialysis Starts
  • Hospitalizations and Readmissions
  • Provider engagement and physician satisfaction
  • Patient engagement

Scope: generally 8-12 direct reports, 2,000-3,000 lives

Travel: generally, 20-50% of the time for physician practice meetings, market / team meetings, and team member shadowing, coaching and mentoring

Qualifications

Qualifications/Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include:

  • Clinical operations background required.
  • Advanced Practitioner degree or Bachelor’s degree in Nursing preferred.
  • Minimum of 5 years of nursing leadership experience with multi-location healthcare preferred; other relevant experience will be considered; previous dialysis experience a strong plus.
  • Proficient with all Microsoft Office products, including Outlook, Word, and Excel
  • Proven experience in management with a focus on team leadership and operational management
  • Strong analytical skills with the ability to interpret performance metrics and implement improvements
  • Excellent communication and interpersonal skills to foster relationships and drive team success
  • Experience in program development and strategic planning
  • Knowledge of Health Information Exchange (HIE) accounts and Electronic Medical Records (EMRs) preferred
