Job summary

U.S. Renal Care in Harlingen, TX is seeking a licensed Advanced Practice Nurse to lead ESRD care, including rounding, patient education, and disease management, under nephrologist supervision.

You will collaborate with dialysis teams, participate in care transitions, and promote CKD education. Nephrology experience preferred; strong communication required.

Qualifications

• Current license to practice as an Advanced Practitioner in the state performing duties.
• Firm understanding of the protocols, policies and operations of a physician practice.
• 2 years clinical work experience; nephrology experience a strong plus.
• Excellent written and verbal communication skills.

Responsibilities

• ESKD rounding under the guidance of practice nephrologists.
• Holistic patient care management including activation, depression, high hospitalization risk, transition of care and comorbidity oversight.
• ESKD patient education.
• ESKD comprehensive disease management annual visits.
• Collaboration with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialists as clinically indicated.
• CKD clinical care and kidney disease education.
• Other duties as assigned.
• Perform physical examinations and evaluate health.
• Order and assess diagnostic tests.
• Determine and initiate appropriate treatment; adjust as needed.
• Prescribe medications.
• Document medical information and EMR.
• Educate and counsel patients and families on health problems and home care.
• Request consultations with other providers; coordinate with Kidneylink RN Care Manager.
• Promote guest customer service standards; build relationships at all levels.
• Participate in team concepts; align with company policies.
• Regular and reliable attendance.

Responsibilities

SUMMARY

Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.

Primary duties include:

• ESKD rounding, including limited and comprehensive visits, under the guidance of practice nephrologists
• ESKD hollistic patient care management including but not limited to patient activation, depression, high risk of hospitalization, transition of care and co-morbid medical issue oversight
• ESKD patient education
• ESKD comprehensive disease management annual visits
• Care collaboration with with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialisits as clinically indicated
• CKD clinical care and kidney disease education
• Other duties and tasks as assigned

Functions performed during above duties:

• Perform physical examinations of patients to evaluate health and diagnose illnesses
• Order and assess diagnostic tests, such as x-rays and laboratory tests
• Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
• Prescribe medications
• Document medical information of patients and maintain electronic medical records
• Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors
• Request consultation or referral with other health care providers, such as physicians and clinics, collaborate with Kidneylink RN Care Manager
• Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
• Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.
• Regular and reliable attendance is required for the job.

Qualifications

Qualifications/Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include:

• Current license to practice as an Advanced Practitioner in the state performing duties
• Ideal candidate will have a firm understanding of the protocols, policies and operations of a physician practice.
• Must have necessary specialty certification 2 years clinical work experience Nephrology experience a strong plus.
