Job summary

Diversicare is seeking a compassionate Certified Nurse Assistant (C.N.A.) to join our team in Alabama. You will accompany residents to medical visits, ensure safety during transportation, and assist with required documentation and follow-up tasks.

Qualified candidates hold an active C.N.A. certification, possess a valid state driver’s license, and maintain a clean driving record. The role emphasizes service excellence and respectful patient care in a skilled nursing setting.

Qualifications

  • Must be a Certified Nurse Assistant (C.N.A.) with current certification.
  • Physically able to assist residents transfers.
  • Valid state driver’s license and clean driving record.

Responsibilities

  • Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
  • Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
  • Observe the following guidelines at each visit: a. Sign in resident upon entrance. b. Present resident’s documents to office personnel.
  • Before leaving doctor’s office, please observe the following: a. Obtain resident’s documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor’s order form. d. If follow-up appointment necessary, obtain appointment cards.
  • After returning to facility, report to RCM and be prepared to answer questions concerning visit.
  • Performs other duties as required and assigned.

Qualifications
  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state driver’s license.
  • Qualified as a Certified Nurse Assistant (C.N.A.).
  • Clean driving record.
  • Clean criminal record.
  • Any other state required certifications
