Job summary

Complete-Health is seeking an experienced Physician Assistant to join our outpatient primary care clinic focused on value-based, relationship-driven care. You will have your own patient panels and work with an interdisciplinary team to keep patients healthy and out of the hospital.

We emphasize time for prevention, education, and partnering with patients to improve long-term outcomes, with a predictable outpatient schedule and autonomy in clinical practice.

Qualifications

• Graduate of an accredited PA program.
• National board certification.
• BLS certification.
• Preferred: primary care or outpatient experience.
• Preferred: chronic disease management experience.

Responsibilities

• Provide comprehensive primary care services including preventive, wellness, and acute visits.
• Diagnose and manage acute and chronic conditions using evidence-based guidelines.
• Develop longitudinal care plans focused on prevention and chronic disease optimization.
• Manage medically complex patients and address social determinants of health.
• Prescribe medications within scope of practice.
• Collaborate with nurses, care coordinators, behavioral health providers, and specialists.
• Use population health data to close care gaps.
• Educate and empower patients to participate in their care.
