Qualifications

• MD or DO degree required and U.S. medical license active.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or Palliative Care.
• Passion for adult primary care and chronic disease management.
• Interest in value-based care and collaborative teams.
• Excellent communication and interpersonal skills.
