Job summary

Oak Street Health seeks a Physician Assistant to deliver effective value-based primary care to adults on Medicare in our outpatient clinics. You will work with a collaborative care team to provide longitudinal care, perform annual wellness visits, coordinate with specialists, and educate patients about their health plans and treatments.

Strong emphasis on building trusting relationships and population health outcomes.

Qualifications

• 3 years primary care experience (preferred).
• Active, non-probationary state Physician Assistant license.
• Active DEA license.
• US work authorization.
• Experience in primary care settings and care coordination.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with providers, specialists and facilities.
• Population health leadership with care team coordination.
• Phone triage and outreach support.
• Educate patients on health conditions and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
