Job summary

Upperline Health is seeking a Primary Care Medical Assistant to support a value-based clinic model. The MA will room patients, record vital signs, collect histories, and assist providers in clinic visits, while coordinating post-visit care and outreach to improve outcomes.

The role emphasizes accurate documentation in the EHR, collaboration with nurses and care managers, and proactive patient engagement in a fast-paced team setting. On-site in Lawrenceville, GA.

Qualifications

• Certified MA or minimum of 2 years’ experience in a medical primary care setting.
• Experience in a primary care /outpatient clinic setting is strongly preferred.
• Phlebotomy certification strongly preferred.

Responsibilities

• Room patients, obtain and document vital signs, medical history, and chief complaints.
• Assist NP with health assessments and in-clinic visits for Upperline Plus patients.
• Coordinate care and support extended care team efforts.
• Document clinical information in the EHR accurately.
• Perform medication reconciliation at patient encounters.
• Telephonic outreach to coordinate preventative care and follow-up.
