Job summary

gobravvo is seeking a Primary Care Physician (MD/DO) in Columbia, SC to join a value-based care network serving Medicare populations. The role emphasizes high-quality, patient-centered care within a collaborative team and modern resources.

Qualifications

• MD/DO degree required.
• Board certification in Internal or Family Medicine preferred or eligible.
• Active, unrestricted state medical license.
• Experience in value-based care and team-based practice preferred.
• Fellowship in geriatrics is a plus.

Responsibilities

• Provide comprehensive primary care for adult and geriatric patients.
• Manage patient panels with defined visit schedules in a value-based model.
• Collaborate with scribes, medical assistants, social workers, and care teams.
• Utilize EMR systems (Canopy, Greenway) for documentation and care coordination.
• Participate in community wellness and preventive medicine initiatives.
